Each dimension is scored on a five-point scale, and every score is justified in one sentence citing evidence.
A score without evidence is an opinion, and the client will treat the whole report that way.

Why comparability matters
Because the same scale is used on every engagement, an annual re-diagnostic shows movement rather than a change of opinion, and results are comparable between one institution and its peer group.
